Uprima (Apomorphine) is a drug designed to help men who suffer from erectile dysfunction. Uprima can help men who suffer from erectile dysfunction achieve and maintain an erection when sexually stimulated. This stimulation can be either physical or visual.

How Uprima works

Uprima (apomorphine) is a dopamine receptor agonist that works in the brain to improve diminished erectile function by enhancing the natural signal to the penis following sexual stimulation, similar to the way men normally have erections. UPRIMA works through the central nervous system, producing a series of events that enhances the ability to achieve and maintain penile erection. It is distinct from other oral therapies, which act by blocking the action of certain enzymes involved in the erectile response. UPRIMA is sublingually administered (under the tongue), and it enters the bloodstream quickly, bypassing the stomach and releasing dopamine, one of the chemicals in the brain responsible for achieving and maintaining an erection. Uprima increases the levels of nitric oxide in the body that dilates blood vessels and increases blood flow to the penis. It also aids in the transmission of the signals from the brain via the spinal cord which also aids blood flow to the penis, helping to achieve an erection.  The rapid absorption of UPRIMA into the bloodstream allows for its fast onset of action, typically under 20 minutes. Because some patients can experience dizziness, lightheadedness and, uncommonly fainting, it is recommended that you do not drive or operate machinery for at least 2 hours after taking UPRIMA or until any such symptoms are fully resolved. Your Doctor will decide the dose of UPRIMA which is suitable for you. Do not take more tablets than your Doctor has prescribed. All patients should begin with the 2 mg tablet. If your erection is not sufficient for satisfactory sexual activity at this dose and you have not experienced side effects, you may increase the dose to the 3 mg UPRIMA tablet following consultation with your doctor. You should not take more than 3 mg of UPRIMA in any 8 hour period.  

Uprima (Apomorphine-EU) Side Effects

In clinical trials, the most commonly reported adverse events were nausea, dizziness and headache, which were generally mild and transient in nature. Do not take Uprima if: (1) You have ever had an allergic reaction to apomorphine or any of the ingredients in Uprima. Possible signs of an allergic reaction include rash, itching, swollen face or lips and shortness of breath. Tell your Doctor if you get any of these. (2) You have severe unstable angina, recent myocardial infarction, severe heart failure or hypotension, and other medical conditions which could make sexual activity dangerous. Always consult your Doctor about your medical history. (3) Your partner is pregnant or breastfeeding. UPRIMA should not be used if (1) You have a deformity or disease of the penis as these may require special care when using medicines such as Uprima. (2) You experience severe dizziness/lightheadedness or feel faint after taking Uprima, especially if you also have severe nausea or vomiting, sweating, become pale or become unusually hot. In these cases you should not attempt to stand up. If you experience these symptoms or feel faint for any reason you should lie down and raise your legs. The feeling of faintness should go away. (3) You are taking nitrates (often used for the relief of angina or chest pains) or any medicines for hypertension (high blood pressure). (4) You have liver or kidney problems. Your suitability for Uprima and/or the appropriate dose may need to be considered by your doctor. (5) You have abnormally high or low blood pressure or are prone to dizzy spells or fainting. (6) You are taking any other treatment for erectile dysfunction. Uprima should not be used with any other treatment for erectile dysfunction. In addition, Uprima should not be given to children under the age of 18. No dose adjustment is required for the elderly. Taking alcohol with Uprima can increase any undesirable effects, in particular low blood pressure. In addition, taking alcohol can make it more difficult to obtain an erection. Alcohol consumption and the use of UPRIMA is NOT RECOMMENDED. UPRIMA is NOT indicated for use in women.

UPRIMA is contra-indicated for patients who (1) Are taking nitrates (often used for the relief of angina or chest pains) or any medicines for hypertension (high blood pressure) and (2) Are taking drugs which affect the dopamine system (often used to treat Parkinson's disease, disturbances of the mind or to prevent vomiting). Accoring to Abbott Labs, UPRIMA has been extensively studied in clinical trials involving more than 5,000 patients, with more than 120,000 doses of UPRIMA administered. UPRIMA was studied in men with varying severity of erectile dysfunction and included patients with organic, psychogenic or mixed etiology erectile dysfunction. The effectiveness of UPRIMA was measured by the ability to produce an erection firm enough for intercourse, with assessments conducted by both patient and partner on a per-attempt basis, a unique approach in ED clinical trials.

Median time to onset of erection is 18-19 minutes

In Uprima responders :

- 34% of erections occurred within 10 minutes
- 71% of erections occurred within 20 minutes

Most common side effects

- headache ( 7% )
- nausea ( 7% )
- dizziness (~4%)

Nausea is generally mild to moderate in nature, declining with continued use.

Who should not take Uprima?

Uprima is only to be taken by men who suffer from erectile dysfunction. Uprima is not for women or children. As with all prescription drugs it should only be taken on prescription and not by anyone else.

Before taking Uprima consult with your doctor to ensure that your heart is healthy. Men who suffer from acute angina, severe heart failure, myocardial infarction or any condition where sexual activity is not recommended should not take Uprima. Men suffering from impaired renal or hepatic functions must first consult with a medical practitioner before considering taking Uprima.
